By a record sheet one understands the summary of certain data according to a fixed pattern on a map for a card index.

Originally it usually concerned maps out of firm cardboard, on which the desired data, frequently into forms, were registered. These maps were then collected according to certain criteria (e.g. alphabetical) in card index boxes. Thus fast accesses to these data one made possible.

The term is used today also for the data records of an electronic data base, although it actually does not concern no more "maps".
