Evidence class is a recommendation class in the evidence-based medicine. Possibly it indicates, whether studies are present, and, as these studies look. The finer the scale is, the more undisputedly becomes the classifications; is the more simply the class held, will handle the simpler the classifications at expense of the force of expression.

Simplified classification

For non-physicians this classification is the most important, and it has particularly in the emergency service an important function. The individual classes are:

Class I:" " " " " secured recommendation, always acceptable, surely and effectively
Class IIa:acceptable and meaningfully (very well proven advantage)
Class IIb:optionally applicably (well proven advantage, therapy alternative for experienced ones)
Class X:undeterminably; no present scientific realizations or the subject of further investigations or neither proven use nor possible damage
Class III:does not indicate, not useful, possibly harmful

Hierarchy of the external evidence

EBM is not on randomized, controlled studies (abbreviation: RCTs of the English Randomized control LED Trial) and Metaanalysen limits. These proved nevertheless as gold standard in the large majority of those questions, with which it concerns to evaluate use and risks of new like old therapies - in particular within the range of the Pharmakotherapie -.

In the evaluation (validating) of the external evidence thus the following order of rank of the epistemological quality of information in descending order results with consideration of medical, technical and statistic methods:

  • Ia gradates: At least a Metaanalyse on the basis of methodically high-quality randomized, controlled studies
  • Ib gradates: at least a sufficiently large, methodically high-quality RCT
  • IIa gradates: at least a high-quality study without randomization
  • IIb gradates: at least a high-quality study of another type, quasi-experimental study
  • Stage III: more than a methodically high-quality not-experimental study
  • Stage IV: Opinions and convictions of outstanding authorities (from clinical experience); Expertenkomissionen; describing studies
  • V gradates: Drop series or one or more expert opinions
